PWE PTA Reimbursement Request

Please fill out this form to request reimbursements from the Prestonwood Elementary PTA.

As a reminder, we cannot reimburse for sales tax except from Costco and Sam's.  

Stores have a form you can fill out at checkout and provide our organization details.

Those are: Tax id 23-7212581 and address: 6525 La Cosa Dr. Dallas, Tx 75248.

Additionally, we can only reimburse gift cards up to $25.
